Clinical Considerations Form
Our founder sat in a waiting room filling out forms for a medical test and was frustrated that the paperwork asked about respiratory condition but did not consider Neuromuscular Disease (NMD)-related breathing/respiratory muscle weakness in the options. She had to write in the white space and explained her breathing weakness to the technicians administering the test. It was through this experience she realized a form would be helpful to hand to any clinician (physician, therapist, test technician, etc.) to provide basic information about respiratory involvement and a summary of what has been known to worsen neuromuscular breathing weakness.
Our form, "Clinical Considerations for Neuromuscular Disease (NMD) Patients" is a result of a collaborative effort between US clinicians, individuals using mechanical/assisted ventilation, and those who have loved ones who use these respiratory muscle aids. With feedback from these stakeholders, we developed a concise form to be shared by the patient to the clinician in the spirit of positive collaboration and to spread awareness that many treatments that help individuals without NMD may unfortunately harm those with NMD.
Select the link below to download, print, and share the form with your clinician. Our hope is that the form is a quick read for busy clinicians and opens a dialogue to prevent common medical errors that occur too often in the NMD community in the US.
