Respiratory Care Information Card
Our Respiratory Care Information Cards have been around since 2015 when our founder began handing them out at Neuromuscular Disease (NMD) patient and family conferences. The cards, developed specifically for those with NMD, have evolved over the years, starting as larger laminated card stock and slowly becoming the smaller, sturdy wallet card configuration they are today. In early 2023, we made some enhancements to the verbiage on the front of the card and added a back side that provides a QR code you can scan with your smartphone to reach our Oxygen Caution web page with additional information.
The card serves as a medical self-advocacy tool to assist in receiving appropriate care for breathing muscle weakness, including critical information about supplemental oxygen and mechanically assisted cough.
Those who reside within the United States (US), Canada, the United Kingdom (UK), South Africa, India, Australia, France, Netherlands, Ireland, or Germany may order 1, 2, or 3 of the cards for FREE. As a small organization with no paid staff, we do not have the human or financial resources to mail our cards worldwide at this time. The cards are available in the English language only at this time. We appreciate your understanding.
