Giving Tuesday Book Giveaway
We are giving back! If you are residing in the United State (US) and you or your loved one lives with a Neuromuscular Disease (NMD), you are eligible to enter our random drawing to win one of four NMD-related books that were generously donated to us in memory of an individual living with Duchenne Muscular Dystrophy (DMD) who passed away earlier this year. The books are in good condition and include:

Management of Patients with Neuromuscular Disease by John R. Bach, MD
Noninvasive Mechanical Ventilation by John R. Bach, MD
Guide to the Evaluation and Management of Neuromuscular Disease by John R. Bach, MD
Essentials of Mechanical Ventilation, Second Edition by Dean R. Hess and Robert M. Kacmarek

Entries for the book drawing will be accepted on the form below from Tuesday, November 30 until 11:59 p.m. Eastern on Friday, December 3rd. Please limit entry to one applicant per household. All qualified applicants will be exported into a numbered spreadsheet, and an online randomizer tool will be used to generate four different numbers that will be selected as the winners. Only one winner will be selected per household. The winning numbers and the four winning individuals will be announced on the Breathe with MD, Inc. Facebook page on Saturday, December 4 (time to be announced on our social media platforms). We will also contact all winners via e-mail. Books will be mailed out to the winners by USPS the following week.
